### Summary
The Medical Instrument Technician (Polysomnography) is located in the Cardiopulmonary Section of the Medical Surgical Specialty Care Line and functions as a team with sleep lab staff and respiratory care practitioners to provide clinical education to patients, family members, physicians, and hospital staff on the use of CPAP/BiPAP for obstructive sleep apnea, central sleep apnea, complex sleep disorders, and other sleep disorders.

### Responsibilities
Work is primarily in support of the activities managed by the Medical Surgical Specialty Care Line. This may involve diagnostic and therapeutic services and/or patient care and education. Additionally, this may include correlating special devices and education for patients in the Special Care Unit and outside facilities that require special devices to assist and support veterans with respiratory failure.
The Polysomnographic Technician must have the ability to organize and manage time effectively, exercise independent judgment, assume responsibility for their own work/actions, and sometimes work under stressful conditions, remain focused, maintain the ability to prioritize, and transition between tasks quickly. They must possess ethical judgment, integrity, honesty, dependability, and accountability.
Major functions include, but are not limited to the following, and requires the employee with or without reasonable accommodation be able to:
* Review and process sleep study consults for verification of proper testing and disposition according to American Academy of Sleep Medicine (AASM) criteria and VA guidelines.
* Process all necessary documentation, consults, or requests regarding sleep studies.
* Educate patients and families on the use of PAP devices, including safety, infection control, and troubleshooting, maintenance, and sleep hygiene.
* Set-up PAP equipment.
* Troubleshooting of PAP equipment, makes adjustments to setting according to protocol, and documents any changes to PAP therapy.
* Perform compliance studies on PAP devices and generate reports for CPRS and providers.
* Participate in presentations regarding education to increase awareness about sleep and health, good sleep habits, drowsy driving, shift work/jet lag, and other sleep disorders.
* Treat Veterans with physical disabilities, injuries, or disease by use of a wide range of techniques, procedures, and equipment.

### Qualifications
This is a developmental position leading to a GS-8 level, performing assignments intended to broaden skills and provide practical experience leading to progressively more complex assignments performed under progressively less supervision. You will ultimately acquire the background necessary to perform at the full performance level of GS-8. Promotion is at the discretion of the supervisor and is contingent upon satisfactory performance, availability of higher level work, and availability of funds.
**Basic Requirements**:
**Citizenship**: Citizenship of the United States.
**English Language Proficiency:** Medical Instrument Technicians must be proficient in spoken and written English.
**Education:**There are no specific educational requirements for this occupation.
**Licensure or Certification:** Licensure or Certification is not required for this occupation; however, it is strongly desirable at GS-6 or above as evidence of possession of the essential knowledge, skills, and abilities. Basic and Advanced Cardiac Life Support certifications are desirable.
Grade Determinations:
**GS-6-Experience**: At least 1 year of experience comparable to the next lower grade level which demonstrates the knowledge, skills, abilities, and other characteristics related to the duties of the positions to be filled. In addition, the candidate must demonstrate the following KSAs:
**Demonstrated Knowledge, Skills, and Abilities**
1. Knowledge of standard, computerized polysomnographs for recording continuous all night physiological data including EEG, EOG, EKG, EMG and respiratory parameters.
2. Knowledge of standard devices such as oximeters, respiratory effort devices, nasal pressure devices, thermistors, thermocouples, continuous positive airway pressure (CPAP) machines, and laboratory computers including the ability to calibrate and operate the equipment and recognize and eliminate recording artifacts.
3. Knowledge of medical terminology and medical record organization to review medical records, sleep study orders, consent forms, medical charts, etc.
4. Knowledge of polysmnographic procedural protocols (including overnight sleep studies, MSLT, MWT, PAP, etc.).
5. Knowledge of basic anatomy and physiology and the international 10-20 electrode placement system in order to conduct the sleep studies.
**GS-7 - Experience:**At least 1 year of experience comparable to the next lower grade level which demonstrates the knowledge, skills, abilities, and other characteristics related to the duties of the positions to be filled. This would be experience which provided the individual with the knowledge, skills, and abilities to independently conduct difficult but standardized polysomnography examinations. In addition, the candidate must demonstrate the following KSAs:
**Demonstrated Knowledge, Skills, and Abilities:**
1. Knowledge of CPAP function and operation to select and fit appropriate PAP patient interface devices.
2. Ability to independently titrate and document PAP level to achieve therapeutic goals.
3. Knowledge of indications, complications, and contraindications of CPAP therapy.
4. Knowledge of Bi-Level pressure titrations.
5. Ability to recognize changes seen during the PSG tracing and identify all stages of sleep. This includes the ability to score and analyze sleep stages in clinical polysomnography recordings.
**GS-8- (Full Performance Level) Experience:** At least 1 year of experience comparable to the next lower grade level which demonstrates the knowledge, skills, abilities, and other characteristics related to the duties of the positions to be filled. This would be experience which provided a knowledge of the more complex procedures as well as knowledge of the uncommon and/or rare sleep disorders. In addition, the candidate must demonstrate the following KSAs:
**Demonstrated Knowledge, Skills, and Abilities:**
1. Advanced knowledge or the International Classification of Sleep Disorders and the ability to recognize uncommon and/or rare sleep disorders.
2. Ability to summarize and report polysomnographic data in narrative form.
3. Knowledge of event characteristics (e.g., respiratory, cardiac, sleep stage, seizures, etc.) and ability to take appropriate action.
4. Ability to provide briefings and orientation to hospital staff including physicians.
